Information for MUA Vendors
In order for the MUA to process payment to you for purchases or services rendered, the following forms are required:
- W-9 and a voucher, click on each to obtain a copy for your convenience. Please feel free to either print them out for completion in order to submit them through U.S. mail, or fax (fax number is (609) 894-4285) or via email email@example.com.
- The third required document is a NJ Business Registration Certificate. This is in accordance with legislation effective September 1, 2004 (P.L. 2004c.57) and applies to both in state and out of state vendors. Again, you may submit this form via email, U.S. postal mail, or fax. If you do not have a NJ Business Registration Form, you can register online at http://www.nj.gov/treasury/revenue/taxreg.shtml. Click on the link and select ‘Register for Tax and Employer Purposes’. Alternatively, download the paper form and instructions at http://www.nj.gov/treasury/revenue/revprnt.shtml.
THE NJ BUSINESS REGISTRATION CERTIFICATE & W-9 WILL BE KEPT IN OUR OFFICE ON FILE AND WILL NEED TO BE PROVIDED ONLY ONCE UNLESS YOUR BUSINESS HAS AN ADDRESS, NAME OR TAX I.D CHANGES. YOU MUST SUBMIT A NEW VOUCHER WITH EACH NEW INVOICE.
